A concise two-phase presentation of a case report: the ER evaluation and diagnosis followed by a more structured diagnose after further detailed physical examination and investigations, involving brief comments upon clues and caveats of the diagnosis, and subsequent most appropriate treatment approach. Two cases are proposed for discussion: a 46 years old man admitted for violent headache, dizziness, rapid heart beatings and short-time loss of consciousness with sphincter relaxation, and a 55 years old hypertensive female patient, with a sudden appearance of dizziness, headache, repetitive vomiting, vertigo, and postural disturbances.Published
