In patients with non ST elevation acute coronary syndrome (ACS) - UA and non ST elevation AMI, major acute cardiovascular events and left ventricular function were evaluated in relation with lack of control of LDL cholesterol, blood glucose and blood pressure and residual vascular risk. 172 patients with ACS were evaluated for risk factors and residual risk in relation with prognosis and left ventricular function. Patients were divided in 3 groups: G3- ACS no control - 60 patients, G2- ACS residual risk 60 patients, G3 ACS control 52 patients with ACS with control of all risk factors. Residual risk included also markers of oxidative stress, metabolic and inflammatory syndrome. In non ST ACS patients, lack of control of LDL cholesterol, blood glucose and blood pressure or presence of markers of residual risk were associated at 5 years of follow up with a significant increased incidence of left ventricular dysfunction, global wall motion score index and significant higher incidence of cardiovascular death, AMI, stroke, readmission for UA and HF in comparison with controlled risk factors patients.
