Evaluarea prin rezonanță magnetică a teratomului ovarian matur chistic gigant – prezentare de caz | [MRI evaluation of giant mature cystic ovarian teratoma – case report]

Abstract

Introduction: Mature cystic ovarian teratoma, the most frequent type of ovarian germ cell neoplasm is a pluripotent malignancy. It is composed of at least two well-differentiated germ cell layers and it usually appears during the reproductive period. Objective: The aim of this paper is to present the case of a female patient, diagnosed at the age of 63 with a giant mature teratoma, originating from the left ovary, with associated extension into the pelvic region and the mesogastrium and subsequent regional mass effect. The patient developed mild symptoms despite the tumoral size and was submitted for surgery, in order to respect the malignant process. A histopathological exam confirmed the diagnosis. Discussions: Nowadays, due to imaging progress, late diagnosis of mature ovarian cystic teratomas larger than 10 cm is unusual. Conclusions: This article raises awareness of the importance of early diagnosis and adequate imagistic approach of mature teratomas, due to their capacity, although rare, of malignant transformation.

Rezumat

Introducere: Teratomul chistic ovarian matur, cel mai frecvent tip de neoplasm germinal, este o tumoră pluripotentă. Aceasta este alcătuită din cel puțin 2 straturi celulare germinale bine diferențiate și apare de obicei în perioada reproductivă. Scop / Obiectiv: Articolul de față își propune să prezinte cazul unei paciente diagnosticate la vârsta de 63 de ani cu un teratom ovarian chistic matur gigant, cu origine în ovarul stâng, extensie spre micul bazin și mezogastru, asociind efect compresiv asupra structurilor învecinate. În ciuda dimensiunilor mult crescute, pacienta a dezvoltat simptomatologie ușoară, fiind redirecționată către operație, cu scopul de a se îndepărta procesul tumoral descris. Examenul anatomopatologic a confirmat diagnosticul. Discuții: În prezent, datorită progresului tehnicilor imagistice, diagnosticul tardiv al unor teratoame ovariene mature chistice, cu diametrul de peste 10 cm, este rar. Concluzii: Acest articol evidențiază importanța diagnosticului precoce și al unui abord imagistic adecvat al teratoamelor ovariene mature, care, deși rar, au capacitatea de transformare malignă
