Orhiepididimita tuberculoasă revelatoare a infecţiei HIV

Abstract

The emergence of AIDS has changed the natural history of tuberculosis which has now become the second most common infection associated with human immunodeficiency virus infection. We present the case of a 34-year-old male patient who was diagnosed with tuberculous orchiepididymitis as the clinical onset of HIV infection. The patient was admitted to the hospital with fever, right scrotal enlargement, and abdominal pain referred to the right lumbar area and right thigh. Laboratory examinations showed an inflammatory syndrome, negative serology for syphilis but positive serology for HIV. The urine examination was normal. Chest X-ray, plain abdominal radiography, and renal ultrasonography were all normal. The inefficiency of initial treatment with gentamicin and ciprofloxacin led to the practice of the right orchiectomy. Histopathological examination revealed the tuberculous etiology and antituberculous therapy was started. The course was favourable but the life prognosis is determined by the infection with HIV.
